**Schedule: Monday-Friday, 8:00am-4:30pm**

Position responsibilities:
1. Able to reliably work independently with minimum structure and supervision.
2. Able to approach and effectively work with homeless individuals in all types of
settings.
3. Provide on-going outreach and in-reach to homeless individuals on the
streets, in encampments, shelters, and other living situations.
4. Advocate for homeless consumer’s needs to be met.
5. Refer consumers to appropriate services.
6. Follow-up with consumers and other agencies to ensure consumer needs
have been addressed.
7. Re-engage or re-establish contact with consumers who have not followedthrough on necessary services.
8. Develop rapport with homeless individuals in order to assess and assist with
their needs.
9. Research and be familiar with available resources and share resources with
appropriate staff.
10.Monitor transition of consumers into follow-up/referral services such as care
management, housing, financial, medical, etc.
11.Respond to consumer’s crisis situations as needed by developing plans and
interventions.
12.Develop and maintain relationships with other homeless agency staff to
facilitate teamwork among the homeless service community.
13.Provide assistance to shelter staff and homeless care managers to assist with
any problems or day-to-day issues.
14.Consistently treat consumers and co-workers with respect.
15.Work within team environment Agency-wide.
16.Insure timely and appropriate documentation as required by Agency
including: notes, activity logs, PATH surveys, discharge forms, etc.
17.Maintain professional and ethical behavior at all times.
18.Develop and maintain community and MHC inter-departmental relations
including prompt follow-up with all contacts.
19.Maintain consumer’s confidentiality at all times.
20.Attend SOAR (SSI, SSDI Outreach, Access and Recovery Trainings) and
complete application, as determined appropriate and necessary by CM
Supervisor or Program Manager.
21.Coordinate & assist w/scheduling and implementation of mobile clinic;
including prompt documentation & follow up with CM’s, nurses, and providers.
22.Transport consumers as needed to assist in linking to resources.
23.Participate in community coordinated events such as Project Homeless
Connect, Homeless Count & SOAR trainings.
24.Collaborate with other agencies.
25.Attend annual PATH network meeting.
26.Attend and participate in community outreach meetings as deemed necessary
by Program Manager and/or Supervisor/Team Leader.

Essential skills and experience:
High school diploma or general education degree (GED).

Essential requirements for the position:
Valid Tennessee Driver’s License
Acceptable Motor Vehicle Report (MVR)
“F” Endorsement
Acceptable Criminal Background Investigation
Personal Automobile Insurance
Cell Phone Preferred
