MEDICAL COORDINATOR Job Description Job Title: Medical Coordinator Location: All Regions Department: Operations Reports to: Area Director and RN FLSA Status: Non exempt Prepared By: HR/KM Prepared Date: 02/2013 Approval Date: Revised Date: 06/2014 Grade: 1 Summary: The Medical Coordinator is responsible for the administrative activities regarding nursing for the group homes. This position takes instructions and is supervised by the Area Director and assists Registered Nurse (RN). Essential Functions and Responsibilities include the following: The following duties and/or responsibilities are not all-inclusive. The Medical Coordinator may be assigned other duties and responsibilities as directed by the Area Director and assists the Registered Nurse (RN).
Filing paperwork in medical charts: MARS, Physicians Orders, nurses notes, and
consumer appointment request form
Schedule consumers physicians appointment weekly
Schedule consumers add on appointments as they are needed
Notify Home coordinators of scheduled appointments
Print out schedule of appointments for office and field staff
Follow up on physician appointments when consumers are DC’d from hospital/ER
Create medical appointment folder that requires current physician orders, copy of
Medicaid, and consumer appointment request form.
Educate transport staff to request proper documentation on physicians orders and
consumer appointment request form to ensure legitimate physicians orders and feedback
Fax current Physicians orders to Pharmacy and on call nurse.
Create new medical charts
Overnight RX triplicate that needs to be filled ASAP by pharmacy
Unpack received meds from pharmacy
Notify Home coordinator of Received meds to schedule pick up
Mail overstock/DC’d meds to complete for credit and disposal
Schedule appointments for SHL, TxHmL, and Foster Care clients as requested by
the RN
Review medical charts for chronological order and updated physicians orders
Request lab results
Make Memos upon request
Monitor drug report from Complete pharmacy
Request refills and quantities from Physicans
Familiarity with prescription requirements: right patient, right dose, right time, right
route and right frequency
Familiarity with drugs such as controlled substance
Familiarity with Diagnosis such as: HTN, Diabetes, Urinary tract infection, MR and
mental health
Other duties as assigned
Knowledge and Skills:
High school diploma preferred or GED.
Minimum 2 years experience performing administrative duties in a business
environment.
Exceptional Excel and Word skills.
Must be proficient at using office equipment.
Must be detail-oriented.
Good communication skills are a must.
Must be a team-player
Must have the ability to multi-task.
Language Skills: Ability to read and comprehend instructions, manuals, and other company documents. Exhibits above average oral communications skills. Position requires excellent phone, business writing and customer service skills. Mathematical Skills: Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals. Reasoning Ability: Ability to apply common sense understanding to carry out detailed involved written or oral communications or instructions. Ability to deal with problems involving unconventional variables in unusual situations.
