in Baxter, MN is seeking a full-time Registered Nurse who demonstrates a superior level of professional integrity to join our growing team. The Registered Nurse position will function as a Qualified Professional who will be responsible for performing duties of training, supervision, and evaluation of the Caregiver/Personal Care Assistant (PCA) staff as well as the oversight and monitoring of client Home Care services.

Essential Functions and Key Responsibilities
Primary oversight and responsibility for clients with complex health care needs.
Provides appropriate teaching and education to clients and families regarding individualized health care needs and health promotion/prevention.
Provides training and ensures competency for the Caregiver/PCA according to the individual needs of the client.
Serves as a resource for other Qualified Professionals regarding the medical health needs of agency clients.
Completes phone and in-home visits to clients according to state guidelines and provides oversight, evaluation, and supervision of both the effectiveness and satisfaction of services.
Creates, implements, and maintains an individualized plan of care related to client needs.
Prepares clinical and progress notes, monitors for change in client condition and assesses for both physical and psychological needs of the client.
Ensures that authorized client services are being provided according to agency standards.
Monitors and ensures that services are being delivered in a caring, compassionate, and respectful manner that exceed industry standards.
Appropriately documents all training, communications, evaluations, and needed actions to improve the performance of our Caregivers/PCA's.
Maintains complete, accurate and timely clinical records/documentation on all clients according to agency policy.
Must be licensed as a Registered Nurse in Minnesota. License must be in good standing, without restrictions, suspensions, or revocations.
Must complete and pass a state required background study.
Must have a valid driver's license and use of insured personal vehicle.
Must be able to lift, turn, position, and transfer up to 75 pounds.
Must demonstrate the ability to read, write and carry out both written and verbal directions in English.
Have at least (2) years of experience in a health care setting or equivalent, home care experience preferred.
Must have the ability to organize workflow and prioritize daily schedule.
Must demonstrate exceptional clinical judgment and can recognize health needs and safety concerns in the home.
Must feel comfortable working with clients of all ages and can adapt to a variety of working environments and community settings.
Full-time 40 hours per week
Business hours Monday thru Friday
Occasional evenings or weekends as necessary to meet deadlines
Paid Time Off
Paid time off for the six major holidays which include New Year's, Memorial Day, Independence Day, Labor Day, Thanksgiving and Christmas.
This position does require travel within the agency's service area.
