As the Referral Coordinator for Crossroads Hospice & Palliative Care, you are the “voice facing” customer service for patients, families and healthcare providers that need our team’s support. In this role, you will respond to an urgent need presented on the referral calls by offering an assessment for patient focused care through a patient focused discussion. Additionally, as the point person for referral calls you will be managing the schedule for nurses who are the first in person interaction families will have about finding the right care at the right time.

Minimum of 6 years of de-escalation, phone customer service experience required.
Excellent conflict resolution skills with the ability to communicate to external and internal customers.
Excellent phone presence as you are the “voice facing” customer service.
Excellent time management skills and follow through with detail oriented tendencies.
Technology and Computer savvy is required as you will be working through multiple systems and providing notes on the conversation to the nursing team.
Responsible for maintaining complete confidentiality of patients' records and all other confidential information which may be received during the course of patient care, including financial information and personnel records.
Confidence to make quick decisions in scheduling the appropriate assessment nurse to visit the patient.
Receive and promptly process referrals and/or inquires for services provided by hospice.
Knowledge of hospice and palliative care
Ability to provide compassionate yet efficient support in order to help patient’s and families or referral sources understand the urgency in having an assessment nurse come visit.
